Job Summary
Physical Therapist for home health, full-time role providing in-home care with travel to patient residences. Responsibilities include evaluating patients, developing and adjusting care plans, delivering skilled PT services (functional assessments, ROM testing, exercise programs), educating patients and caregivers, supervising PTAs and HHAs, maintaining documentation, participating in interdisciplinary teams, and ensuring HIPAA and infection control compliance. Must be licensed in the practicing state, have CPR certification, strong communication and organizational skills, reliable transportation, and ability to travel to patient homes; willing to work occasional weekends/holidays. Based in Las Cruces, NM with field-based responsibilities.
Required Qualifications
- Graduate of an accredited Physical Therapy program
- Licensed Physical Therapist in the state of practice (required)
- Current CPR certification
- Strong communication, documentation, and organizational skills
- Reliable transportation and ability to travel to patient homes
- Must be able to lift and transfer patients, adapt to schedule changes, and work occasional weekends or holidays
- Minimum of 2 years’ experience preferred (PT/PTA)
- Travel to patient homes

- Infection control policies
- Provide skilled physical therapy services including functional assessments, ROM testing, exercise programs
- Supervise Physical Therapy Assistants and Home Health Aides as required
- Evaluate patients and assist physicians in developing plans of care
- Maintain timely and accurate clinical documentation
- Participate in interdisciplinary team meetings and continuous education
- Comply with agency policies
- Graduate of accredited PT program
- Licensed PT in state of practice
