Job Summary
Conduct comprehensive skilled physical therapy services in patient homes as ordered by attending physicians, including initial evaluations, OASIS assessments, and plan of care development. Educate patients and families on disease management, equipment use, and preventative measures to decrease re-hospitalizations. Document all treatment aspects, assessments, and patient education within specified timeframes and ensure compliance with legal regulations. Supervise therapy services provided by PTAs and nurse assistants when appropriate. Drive to assigned visit locations and participate in on-call rotations and case conferences.
Required Qualifications
- Physical Therapist with a current license in the state of employment
- Minimum of one year of experience as an PT in an acute setting
- Valid driver's license and auto insurance with your name as a listed driver
- Must be able to meet all physical position requirements
- Evidence required of a current negative tuberculin skin test or x-ray or receive TB testing at the date of orientation
- Annual tuberculin skin test requirement thereafter unless documentation of previous TB test exists
- Must be free of physical/medical conditions which would limit or restrict ability to perform job functions
- Travel is required for this position
