Job Summary
Conduct comprehensive home assessments of patient functional status, mobility, and safety within residential environments. Develop and implement individualized plans of care focusing on real-world skills like fall prevention, gait training, and home exercise program education. Collaborate with an interdisciplinary team including RNs, OTs, and physicians to coordinate holistic recovery approaches. Maintain timely and accurate progress notes in electronic medical records in accordance with CMS regulations. Requires an active Oregon PT license, BLS certification, and valid driver's license with proof of auto insurance. Full benefits package and competitive salary offered.
Required Qualifications
- Active, unencumbered Physical Therapist (PT) license in the state of Oregon
- Graduate of an accredited Physical Therapy program
- Current BLS (American Heart Association) certification
- Valid driver's license
- Reliable transportation
- Proof of auto insurance
Desired Qualifications
- 1+ year of clinical PT experience
- Home health experience
- DPT degree
