Job Summary
Provide one-on-one physical therapy to older adults in their homes or senior living communities (private residences, Independent Living, Assisted Living, Memory Care). Conduct evaluations, develop individualized plans of care, and deliver evidence-based treatment to improve mobility, independence, and function. Deliver therapy sessions, support wellness and aging in place, and document in FOX’s EMR. Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists), educate patients, caregivers, and staff, and offer telehealth when appropriate to extend access. Create and manage your own treatment schedule with opportunities to work in-home and within senior living settings. Qualifications include a valid PT license, degree from an accredited PT program, CPR certification, EMR proficiency, and ability to drive to patient homes. Desirable skills include telehealth components and willingness to treat older adults across various living settings.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Current CPR certification
- Basic computer and EMR proficiency
