Job Summary
Evaluates and conducts medically prescribed physical therapy treatment programs to treat movement dysfunction and pain resulting from injury, disease, disability, or other health-related conditions. PRIMARY RESPONSIBILITIES: Performs initial and ongoing assessments of patient’s condition; establishes, revises and evaluates a plan of care involving the patient/family; performs therapy interventions utilizing standard physical therapy techniques; provides education to patient/family/staff by assessing educational needs; ensures continuity of care through discharge planning, home instructions, equipment/home modifications, and follow-up care recommendations; oversees activities of physical therapy assistants, students, and other support personnel; maintains documentation detailing patient response to therapy and need for skilled intervention as outlined by regulatory bodies; works in a safe, alert manner; performs other duties as assigned. EDUCATION: Bachelor’s, Master’s or Doctorate in Physical Therapy, or related as required to obtain licensure in state of practice. EXPERIENCE: No experience required. PHYSICAL REQUIREMENTS: includes standing, lifting, transferring, repetitive motion, and occasional driving. LICENSES/CERTIFICATIONS: BLS HCP within 90 days; state licensure approvals for IL, MO, OK, WI.

- Basic Life Support Healthcare Provider (BLS HCP) within 90 days of employment
