Job Summary
Evaluates and conducts medically prescribed physical therapy treatment programs to address movement dysfunction and pain due to injury, disease, disability, or other health-related conditions. Performs initial and ongoing assessments, develops and revises plans of care with patient/family, delivers therapy interventions using standard PT techniques, and provides education to patients, families, and staff. Ensures continuity of care through discharge planning, home instructions, equipment/home modification recommendations, and follow-up care. Oversees physical therapy assistants, students, and other support personnel; maintains accurate documentation of patient responses and interventions compliant with regulatory bodies; adheres to safety requirements; and participates in departmentally coordinated patient care across multiple practice locations requiring Illinois, Missouri, Oklahoma, and Wisconsin licensure.
Required Qualifications
- Resuscitation Certification Requirement: Basic Life Support Healthcare Provider (BLS HCP) within 90 days of employment
- Licensed Physical Therapist in IL, MO, WI, and OK per respective regulatory boards
- Ability to obtain/maintain required state licensure to practice physical therapy in IL, MO, OK, WI
- Documentation of licensure to practice physical therapy as required by state boards
