Job Summary
Provide one-on-one physical therapy to older adults in private residences or Senior Living Communities, delivering evaluations, individualized plans of care, and evidence-based treatment. Deliver therapy to support wellness, mobility, and independence; document in FOX’s EMR; collaborate with an interdisciplinary PT/OT/SLP and Fitness Specialists; educate patients, caregivers, and staff to promote aging in place; offer telehealth/virtual care when appropriate; in-home, community-based care with opportunities for in-person and remote delivery; multiple employment types (Full-time, Part-time, PRN/Flex) with H1B sponsorship available; drive to patient homes and meet regulatory requirements.
Required Qualifications
- Valid Physical Therapy license in the state of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Current CPR certification
