Job Summary
Provide one-on-one physical therapy to older adults in private residences and senior living communities, performing evaluations, developing individualized plans of care, and delivering evidence-based treatment to support mobility and functional independence. Document sessions effectively within the EMR system, collaborate with interdisciplinary teams, and educate patients and caregivers on safety and aging in place. Incorporate telehealth and virtual care components as appropriate to extend access and flexibility. This role requires a valid Florida PT license, EMR proficiency, and the ability to lift up to 50 lbs occasionally while driving to patient locations. Full-time, part-time, and PRN employment types are available in Spring Hill, FL.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Current CPR certification
- Basic computer and EMR proficiency
- Must be able to exert medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Must be able to perform CPR
- Must be able to provide moderate to maximum assistance with patient transfers and bed mobility
- Must be able to reposition patients, stand/walk for extended periods, climb stairs, and perform therapeutic activities
- Must be able to drive to patient homes and senior living communities as part of the Geriatric House CallsTM model
- Must be able to wear PPE when required
- Must be cleared through the Florida Background Check Clearinghouse
