Job Summary
Provide one-on-one physical therapy to older adults in-home or within senior living communities, conducting evaluations, developing individualized plans of care, delivering evidence-based treatment, and documenting in FOX’s EMR; collaborate with an interdisciplinary team; educate patients, caregivers, and staff; and offer telehealth/virtual care when appropriate; multiple employment types available including full-time, part-time, and PRN with H1B sponsorship for qualified clinicians; New Graduates eligible for mentorship program.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
