Job Summary
Physical Therapist position for FOX Rehabilitation in Somerset, NJ, delivering one-on-one in-home and senior-living care to older adults. Responsibilities include evaluations, developing and delivering individualized treatment plans, conducting therapy sessions to promote mobility and independence, documenting in FOX’s EMR, collaborating with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists), educating patients and caregivers, and providing telehealth/virtual care when appropriate. Required Qualifications
- Licensed Physical Therapist in the state of practice (or eligibility to obtain)
- Degree from an accredited Physical Therapy program
- Current CPR certification
- EMR/data-entry proficiency or experience
- Ability to drive to patient homes and community settings
- Ability to perform physical tasks and patient transfers as required
