Job Summary
Provide comprehensive skilled physical therapy services and interventions per physician orders, including start of care evaluations, resumption of care, recertifications, and discharges. Assess patient needs, plan interventions, evaluate outcomes, and modify care to meet goals while collaborating with the interdisciplinary team. Educate patients, families, and caregivers on therapy programs and home exercise regimens. Requires a valid California Physical Therapy license, current CPR certification, and driver's license. Required Qualifications
- Active license/registration with the Physical Therapy Board of California
- Graduate of an accredited Physical Therapy program
- Strong documentation and organizational skills
- Excellent communication and interpersonal abilities
- Willingness to travel to patient homes for evaluations and treatment
- Flexibility regarding work locations throughout our service area
- Valid California Physical Therapy license
- Current CPR certification
- Valid California driver's license
- Current California auto insurance
- Completed physical examination, TB test, and required vaccines/titers
- Two professional references
