Job Summary
Perform physical therapy evaluations and develop individualized treatment plans focused on mobility, strength, balance, gait, transfers, and endurance. Monitor resident progress to adjust clinical strategies, while educating residents and caregivers on exercises and safety techniques. Collaborate with PTAs, OTs, SLPs, and nursing staff to coordinate care, and complete accurate clinical documentation. This full-time role at West River Civita Care Center in Milford, CT offers a $45–$50/hour salary with benefits including medical, dental, vision, and 401(k).
Required Qualifications
- Active Connecticut Physical Therapist license
- Graduate of an accredited Physical Therapy program
- Strong clinical skills
