Job Summary
Complete in-home physical therapy evaluations and develop individualized treatment plans to improve patients' mobility, strength, balance, and function. Educate patients and caregivers while coordinating care with physicians and clinicians. Maintain accurate clinical documentation for all visits. This role requires an active Florida PT license and strong assessment skills, with home health experience preferred.
