Physical Therapist – Sioux City, IA

Physical Therapist – in-home and senior-living care focused on delivering one-on-one geriatric therapy to older adults in private residences or senior living communities. Responsibilities include performing evaluations and developing individualized care plans, delivering evidence-based therapy sessions, documenting in the EMR, collaborating with an interdisciplinary team, educating patients and caregivers to promote safety and aging in place, and offering telehealth/virtual care when appropriate. Must have a valid PT license, be able to meet physical demands of in-home care, and be comfortable driving to patient homes.
