Physical Therapist (PT) needed for in-home and senior living care in Springfield, IL, with a sign-on bonus for qualified clinicians. Responsibilities include one-on-one physical therapy with older adults in private residences and senior living communities, evaluations, development of individualized plans of care, evidence-based treatment, documenting in FOX’s EMR, interdisciplinary collaboration, patient and caregiver education, and extending access via telehealth/virtual care when appropriate.
