Job Summary
Provide one-on-one physical therapy to older adults in private residences and senior living communities. Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment supporting mobility and functional independence. Document sessions within FOX's EMR system and collaborate with interdisciplinary teams including OTs, SLPs, and fitness specialists. Educate patients and caregivers on safety and aging in place. Incorporate telehealth and virtual care when appropriate to extend access. Manage your own treatment schedule with flexibility to drive meaningful patient outcomes.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
- Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Performing CPR
- Providing moderate to maximum assistance with patient transfers and bed mobility
- Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
- Driving to patient homes and senior living communities as part of the Geriatric House CallsTM model
- Wearing PPE when required
