Job Summary
Provide one-on-one physical therapy to older adults in-home or within senior living communities, performing evaluations, developing individualized plans of care, and delivering evidence-based treatment to improve mobility, independence, and safety. Document effectively in FOX’s EMR, collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists), educate patients, caregivers, and staff to promote aging in place, and offer telehealth/virtual care when appropriate to extend access and flexibility.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
