Job Summary
Physical Therapist role in Greenwood, IN with FOX Rehabilitation delivering one-on-one geriatric physical therapy in private residences and senior living communities, with telehealth/virtual care optional to extend access. Responsibilities include evaluations, developing plans of care, delivering evidence-based treatment, documenting in FOX’s EMR, collaborating with an interdisciplinary team, educating patients and caregivers, and promoting safety and aging in place. Flexible in-home and senior-living settings, autonomy to manage treatment schedules, and opportunities for telehealth integration are highlighted. Requirements include licensure or eligibility to practice in Indiana, CPR certification, EMR proficiency, and the ability to perform the physical demands of in-home care.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Current CPR certification
- Basic computer and EMR proficiency
- Ability to meet physical demands of in-home and community-based care (e.g., lifting up to 50 lbs occasionally, long periods of standing/walking)
