Physical Therapist (Sign on Bonus)

Physical Therapist delivering geriatric home- and community-based rehabilitation, performing comprehensive evaluations, developing and implementing individualized treatment plans, and using outcome measures to guide clinical decisions. Interventions include therapeutic exercise, balance and gait training, fall prevention, pain management, functional mobility training, and caregiver education. Collaborates with an interdisciplinary team, maintains accurate documentation in EMRs, manages a caseload, and supports patient-centered care with autonomy in a field/on-site environment.
