Job Summary
Provide one-on-one physical therapy to older adults in private residences and senior living communities as part of FOX Rehabilitation's Geriatric House Calls model. Deliver evaluations, develop individualized plans of care, and administer evidence-based treatment to improve mobility, independence, and quality of life. Telehealth/virtual care may be incorporated to extend access. Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists), educate patients and caregivers, document in FOX’s EMR, and support aging-in-place initiatives. Roles include in-home and senior living settings with flex schedules and clinician-driven opportunities. H1B sponsorship available for qualified clinicians; new graduates may participate in FOX’s Emerging Professionals Mentor Program. Required Qualifications
- Licensed Physical Therapist in state of practice or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Current CPR certification
- Ability to meet physical demands of in-home care, including lifting and transfers
