Job Summary
Provide one-on-one physical therapy to older adults in-home or within senior living communities, including private residences, memory care, and other settings. Conduct evaluations, develop individualized plans of care, and deliver evidence-based therapy to improve mobility, function, and safety. Document in FOX’s EMR, collaborate with PT/OT/SLP and fitness staff, educate patients, caregivers, and staff, and offer telehealth/virtual care when appropriate to extend access. Roles include full-time in-home/senior living care, part-time, and PRN options, with H1B sponsorship available for qualified clinicians and new graduates eligible for FOX’s Emerging Professionals Mentor Program.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
