Job Summary
Sign-on bonus available for a Physical Therapist providing in-home, senior living, and telehealth care through FOX Rehabilitation. Deliver one-on-one PT to older adults in private residences and senior living communities, performing evaluations, developing individualized plans of care, and delivering evidence-based treatment to improve mobility, independence, and quality of life. Schedule is flexible with autonomy to tailor treatment; telehealth components may be incorporated. Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists), educate patients, caregivers, and staff to promote safety and aging in place, and document in FOX’s EMR. Roles include in-home and community settings with opportunities across full-time, part-time, and PRN/flex paths; H1B sponsorship is available for qualified clinicians. New graduates are eligible for the Emerging Professionals Mentor Program, and benefits include medical/dental/vision, 401(k) match, continuing education, and professional development resources.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
