Physical Therapist

Conduct comprehensive physical therapy evaluations based on MD orders, develop treatment plans with long and short-term goals, and deliver modalities while modifying care as indicated. Document findings, daily progress, and updated treatment plans accurately on standardized forms, discharge summaries, and MDS care plans. Attend facility and rehab meetings, participate in interdisciplinary team scheduling, and perform patient screens. Supervise junior staff or students, co-sign notes, and complete required billing for services rendered. Cover other therapists' caseloads during absences and provide inservice education to staff and community partners. Adhere to productivity requirements, scheduled hours, and facility policies while maintaining professional relationships with patients and families.
