Job Summary
Assess, plan, and organize rehabilitative programs to improve functional mobility, relieve pain, and correct disabling conditions from disease or injury. Provide age-appropriate education for clients and caregivers while supervising care provided by team members. Perform modalities and therapeutic interventions per accepted standards, select appropriate assessment procedures, and establish formal plans of care addressing individual patient needs. Initiate collaborative interactions with internal and external team members, patients, and family members to improve outcomes. This role supports Sentara Health's mission to improve health every day within a diverse, not-for-profit integrated healthcare provider serving over 30,000 members.
Required Qualifications
- Bachelor's Degree
- Doctorate Degree in Physical Therapy
- Required license must be valid in state of practice
- Performs modalities/therapeutic interventions in accordance with accepted standards of care and department policies and procedures
- Provides effective instruction to the patient and family through standardized educational programs and/or individualized training
- Initiate, develop, and maintain collaborative interactions with internal and external team members, patients, and family members to improve patient outcomes
- Selects and performs the appropriate assessment procedures
- Establishes and updates formal plan of care to address individual patient needs
