Job Summary
Screen residents for therapy needs, evaluate patients referred by approved sources, and develop initial and ongoing treatment plans. Supervise Physical Therapist Assistants and Rehab Techs during group and individual treatments, providing clinical guidance and ensuring safe biomechanical lifting of up to 50 pounds. Manage discharge planning, coordinate interdisciplinary team interactions, and maintain accurate billing and documentation in compliance with state and federal regulations. Travel between communities and work varying shifts including weekends and holidays.
Required Qualifications
- Valid physical therapist state license (where applicable)
- Graduate of an APTA accredited school of physical therapy
- Proficient knowledge of Microsoft Office (Outlook, Word, Excel, Power Point, etc.)
- Basic Internet navigation skills
- Must be able to travel between communities during scheduled hours
- Must be able to work varying hours and shifts, including weekends and holidays
- Must be capable of lifting fifty (50) pounds of dead weight alone
- Must maintain valid state license (where appropriate)
- Must conduct self in an ethical, legal, and responsible manner at all times
- Must adhere to the policies, principles, and guidance within the Team Member Handbook and Code of Business Conduct
- Ability to read, write, speak, and comprehend in English: instructions, correspondence, charts, memos, and reports
- Ability to work in a fast-paced, productive work environment
- Must be able to lift 50 lbs
