Job Summary
Screen residents, evaluate patients referred by approved sources, and develop initial and ongoing treatment plans while managing discharge planning and case management. Direct and supervise Physical Therapist Assistants and Rehab Techs during group and individual treatments, ensuring compliance with state practice acts and Reliant Rehabilitation policies. Build the physical therapy program by integrating services into the community, contributing to utilization reviews, and establishing relationships with interdisciplinary teams. Maintain accurate billing documentation, adhere to infection control and safety protocols, and travel between communities as scheduled.
Required Qualifications
- Valid physical therapist state license (where applicable)
- Graduate of an APTA accredited school of physical therapy
- Proficient knowledge of Microsoft Office (Outlook, Word, Excel, Power Point, etc.)
- Basic Internet navigation skills
- Must be able to lift fifty (50) pounds of dead weight alone
- Must be able to travel between communities during scheduled hours
- Must be able to work varying hours and shifts, including weekends and holidays
- Attends all mandatory meetings, trainings, and assignments as delegated
- Ability to read, write, speak, and comprehend in English: instructions, correspondence, charts, memos, and reports
- Ability to work in a fast-paced, productive work environment
- Must conduct self in an ethical, legal, and responsible manner at all times
- Must adhere to the policies, principles, and guidance within the Team Member Handbook and Code of Business Conduct
