Job Summary
Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, memory care); perform evaluations, develop individualized plans of care, and deliver evidence-based treatment; document in FOX’s EMR; collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists); educate patients, caregivers, and staff; offer telehealth/virtual care when appropriate to extend access and flexibility; contribute to autonomous in-home and senior living care schedules; drive to patient homes and adapt to in-home and community-based settings.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Current CPR certification
