Job Summary
Conduct thorough physical therapy evaluations and develop individualized treatment plans to improve residents' mobility, strength, and functional independence. Deliver evidence-based therapeutic interventions while documenting all patient care in accordance with state, federal, and facility guidelines. Collaborate with nursing, therapy, and medical teams to ensure coordinated care, educate families on safety strategies, and contribute to ongoing plan-of-care updates. This full-time role at Santa Fe Care Center offers a starting rate of $60/hour with flexible scheduling options, including potential PRN or part-time arrangements, within a supportive team environment focused on helping residents achieve their highest level of independence.
Required Qualifications
- Valid New Mexico state license as a Physical Therapist (PT)
- Degree in Physical Therapy from an accredited program
- Strong communication skills
- Passion for patient-centered care
