Job Summary
Conduct thorough physical therapy evaluations and develop individualized treatment plans to improve mobility, strength, and functional independence for residents. Deliver evidence-based therapeutic interventions while documenting all patient care in accordance with state, federal, and facility guidelines. Collaborate with nursing, therapy, and medical teams to ensure coordinated care and participate in care conferences for ongoing plan-of-care updates. Educate residents and family members on exercise programs, safety, and functional strategies. This full-time role at Santa Fe Care Center offers a starting rate of $60 per hour with flexible scheduling options, including potential PRN or part-time arrangements, within a supportive team environment dedicated to helping residents achieve independence.
Required Qualifications
- Valid New Mexico state license as a Physical Therapist (PT)
- Degree in Physical Therapy from an accredited program
Desired Qualifications
- No experience with skilled nursing rehabilitation, supportive team to mentor and guide
