Job Summary
Provide one-on-one physical therapy to older adults in private residences or within senior living communities as part of FOX Rehabilitation’s Geriatric House Calls model. Responsibilities include performing evaluations, developing individualized plans of care, delivering evidence-based treatment, delivering therapy sessions to improve mobility and functional independence, documenting in FOX’s EMR, collaborating with an interdisciplinary team, and educating patients, caregivers, and staff. Telehealth/virtual care may be incorporated to extend access and flexibility.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Current CPR certification
- Basic computer and EMR proficiency
