Job Summary
Conduct one-on-one physical therapy for older adults in private residences, senior living communities, and memory care settings. Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment to support mobility, independence, and functional longevity. Document sessions within FOX's EMR system and collaborate with interdisciplinary teams including OTs, SLPs, and Fitness Specialists. Educate patients and caregivers on safety and aging-in-place strategies while providing telehealth or virtual care when appropriate to extend access. Manage your own treatment schedule with flexibility to create and adjust visits as needed.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
- Ability to exert medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Ability to perform CPR
- Ability to provide moderate to maximum assistance with patient transfers and bed mobility
- Ability to reposition patients, stand/walk for extended periods, climb stairs, and perform therapeutic activities
- Ability to drive to patient homes and senior living communities as part of the Geriatric House Calls model
- Ability to wear PPE when required
- Must be available for weekend shifts
