Physical Therapist

Screen and evaluate patients with cognitive and/or ADL physical functioning disorders to develop appropriate plans of care. Provide rehabilitative, skilled, and medically necessary interventions including ADL retraining, therapeutic exercises, modalities, and assistive device use. Administer procedures such as wound care, sensory therapy, and group therapy while assessing progress and adjusting treatments accordingly. Consult with patients, families, and caregivers regarding physical disorders and communicate treatment plans with the health team. Supervise physical therapist assistants, rehabilitation aides, and students while documenting all services in the electronic system. Participate in patient care conferences, utilization reviews, and required trainings.
