Job Summary
Provide one-on-one physical therapy to older adults in homes and senior living communities, including private residences and telehealth options when appropriate. Conduct evaluations, develop individualized plans of care, deliver evidence-based treatments to improve mobility, independence, and aging-in-place outcomes. Document in EMR per regulatory standards, collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists), educate patients and caregivers, and offer flexible scheduling with in-home and telehealth capabilities.
Required Qualifications
- Licensed Physical Therapist in the state of practice (Virginia) or eligibility to obtain license
- Degree from an accredited Physical Therapy program
- CPR certification
- Proficiency with EMR systems
- Current ability to drive to patient homes and communities (per in-home/Community-based care)
