Job Summary
Provide one-on-one physical therapy to older adults in private residences, senior living communities, and via telehealth to improve mobility and functional independence. Deliver sessions supporting patient wellness and longevity while documenting effectively within FOX's EMR system. Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists to create personalized care plans. Manage your own treatment schedule with autonomy, incorporating virtual care components when appropriate to extend access. Drive to patient homes and communities as part of the Geriatric House Calls model, meeting physical demands including patient transfers. Join a clinician-operated practice rooted in meaningful relationships and evidence-based care, supported by professional certification reimbursement and mileage allowances.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Must be able to meet the physical demands of in-home and community-based care, including: Providing moderate to maximum assistance with patient transfers and bed mobility
- Must be able to meet the physical demands of in-home and community-based care, including: Driving to patient homes and senior living communities as part of the Geriatric House Calls model
