Job Summary
Provide one-on-one physical therapy to older adults in in-home and senior living settings (private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care) as part of FOX Rehabilitation’s Geriatric House Calls model. Deliver therapy to improve mobility, independence, and functional outcomes, with autonomy to create and manage your own treatment schedule. Utilize telehealth/virtual care when appropriate to extend access and flexibility, document effectively in FOX’s EMR, and collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists). Driving to patient homes is required. New Graduates eligible for FOX’s Emerging Professionals Mentor Program. H1B sponsorship available for qualified clinicians. Compensation includes salaried positions with benefits (Full-Time in-home and senior living care; Part-Time and PRN options available). Mileage reimbursement and opportunities to earn above quota. Valid PT license in CA or eligibility to obtain, degree from an accredited PT program, and strong computer/EMR skills are required.
Required Qualifications
- Valid Physical Therapy license in the state of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Ability to drive to patient homes and senior living communities as part of the Geriatric House Calls model
