Deliver one-on-one physical therapy to older adults in private residences and senior living communities, supporting mobility, independence, and functional wellness. Document sessions effectively within FOX's EMR system and collaborate with interdisciplinary teams including OTs, SLPs, and Fitness Specialists. Incorporate telehealth and virtual care components to extend access and maintain continuity of care. Drive to patient homes and communities as part of the Geriatric House Calls model, providing moderate to maximum assistance with transfers and bed mobility. Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Must be able to meet the physical demands of in-home and community-based care, including: Providing moderate to maximum assistance with patient transfers and bed mobility
- Must be able to drive to patient homes and senior living communities as part of the Geriatric House Calls model
