Job Summary
Co-leads in an interdisciplinary approach to patient groups, provides evaluations and therapy interventions, establishes individualized discharge plans, and ensures communication with appropriate parties within Adventist Health Mendocino County’s coastal Fort Bragg facility.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree: Required; only if obtained license on or after January 2016
- Physical Therapy licensure in all applicable states of practice: Facility Specific License/Certifications
- Physical Therapy licensure in state of practice: Completes evaluations to ensure quality standards and addresses assessment areas that require attention. Provides therapy interventions using appropriate treatment protocols and techniques. Establishes individualized care and discharge plans and ensures plans are communicated to appropriate parties.
