Physical Therapist ~ PT

Provide an individualized physical therapy program including assessment, planning, implementation, education, and communication to maximize patient progress toward functional goals. Collaborate with other health professionals, supervise staff as needed, document evaluations and treatment plans, and maintain HIPAA-compliant records. Role requires direct patient care in a skilled nursing/memory care setting with adherence to standards of care, infection control, and safety policies; opportunities for professional growth, ongoing education, and participation in community outreach.
