Job Summary
Provide skilled physical therapy in patient homes as ordered by attending physicians, including initial evaluations within 48-72 hours of referral. Conduct OASIS assessments to develop individualized plans of care, adjust treatments based on patient response, and supervise PTAs and nurse assistants when appropriate. Educate patients and families on disease management, equipment use, and preventative measures to decrease re-hospitalizations. Complete all clinical documentation, chart audits, and subsequent visits within specified company requirements while maintaining active licensure. Drive to assigned locations and participate in mandatory on-call rotations and case conferences.
Required Qualifications
- Physical Therapist with a current license in the state of employment
- Minimum of one year of experience as an PT in an acute setting
- Valid driver's license and auto insurance with your name as a listed driver
- Must be able to meet all physical position requirements
- Evidence required of a current negative tuberculin skin test or x-ray or receive TB testing at the date of orientation
- Annual tuberculin skin test requirement thereafter
- Employee must be free of physical/medical conditions which would limit or restrict their ability to perform the job functions
