Job Summary
Deliver one-on-one physical therapy to older adults in private residences and senior living communities, supporting mobility, independence, and functional wellness. Document sessions effectively within FOX's EMR system while collaborating with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists. Incorporate telehealth and virtual care components to extend access and maintain continuity. Manage your own treatment schedule with autonomy, providing moderate to maximum assistance with patient transfers and driving to client locations as part of the Geriatric House Calls model.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Must be able to meet the physical demands of in-home and community-based care, including providing moderate to maximum assistance with patient transfers and bed mobility
- Must be able to drive to patient homes and senior living communities as part of the Geriatric House Calls model
- Must be available for weekend shifts
- Must be able to lift 50 lbs
Desired Qualifications
- Experience with telehealth and other forms of virtual care
- Experience collaborating with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
- Experience providing one-on-one physical therapy to older adults
- Experience delivering therapy sessions that support patient wellness, longevity, mobility, and functional independence
- Experience documenting effectively within an EMR system in accordance with regulatory standards
- Experience working in private residences or within Senior Living Communities
- Experience helping patients improve mobility, independence, and quality of life
- Experience creating and managing your own treatment schedule
- Experience building strong, lasting patient relationships
- Experience with Order to Cash, Procure to Pay, Supply Chain, Customers, or Suppliers
- Experience with Risk Identification, Business Acumen, Time Management, or Stakeholder Management
