Physical Therapist PT (Urgent Need)

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain highest levels of physical function. Assess and document patients' physical strengths to support physician evaluations and guide treatment goals. Develop and implement rehabilitation programs consistent with physician plans of treatment and patient team objectives. Provide clear written care instructions to Physical Therapist Assistants and Home Health Aides, overseeing their performance and documentation. Document patient observations, interventions, and evaluations promptly, then report progress to the physician, Clinical Manager, and care team while submitting required evaluations, plans, progress notes, and discharge summaries. Requires a current, unrestricted Physical Therapy license and one year of experience. Full-time employment at $76.00-$106.00 per visit; valid driver's license and personal vehicle liability insurance required.
