Physical Therapist PT (Urgent Need)

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain highest levels of physical function. Assess and document patient strengths to guide treatment goals, then develop and implement rehabilitation programs consistent with physician plans. Provide clear written care instructions to PTAs and Home Health Aides while overseeing their performance and documentation. Report patient progress to the physician and care team, submitting all required evaluations, plans, progress notes, and discharge summaries promptly. Requires a current unrestricted Physical Therapy license and one year of experience. Full-time role with $76.00–$106.00 pay per visit/unit and a $10,000 sign-on bonus.
