Physical Therapist PT (Urgent Need)

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain highest levels of physical function. Assess and document patients' physical strengths to support physician evaluations and guide treatment goals. Develop and implement rehabilitation programs consistent with physician plans and patient team objectives. Provide PTAs and Home Health Aides with clear written care instructions while overseeing their performance and documentation. Report patient progress to the physician, clinical manager, and family, submitting all required evaluations, plans, and discharge summaries. Requires a current, unrestricted Physical Therapy license and one year of experience. Full-time employment at 40 hours per week with a $10,000 sign-on bonus and relocation assistance. Valid state driver's license and personal vehicle liability insurance required as part of Humana's driver safety program.
