Physical Therapist PT (Urgent Need)

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain the highest level of physical function. Assess and document patients' physical strengths to support physician evaluation and guide treatment goals. Develop and implement a rehabilitation program consistent with the physician's Plan of Treatment and patient team objectives. Provide clear written care instructions to PTAs and Home Health Aides, overseeing their performance and documentation. Report patient progress to the physician, Clinical Manager, and care team, submitting all required evaluations, plans, and discharge summaries. Requires a current Physical Therapy license and one year of experience. Pay ranges from $68.00 to $95.00 per visit/unit with a valid driver's license and personal vehicle insurance.
