Physical Therapist PT (Urgent Need)

Plan and administer prescribed physical therapy treatment in patient homes to restore strength, mobility, and independence. Assess physical strengths to guide physician evaluation and rehabilitation programs, while developing and implementing care plans consistent with the physician's treatment plan. Provide clear written care instructions to PTAs and aides, oversee their performance, and document observations, interventions, and evaluations promptly. Report patient progress to the physician, clinical manager, and care team, submitting all required evaluations, plans, progress notes, and discharge summaries. Requires a current, unrestricted Physical Therapy license and one year of clinical experience. Full-time employment with a valid driver's license and personal vehicle insurance.
