Physical Therapist PT (Urgent Need)

Job Summary
Provide in-home physical therapy to patients to regain strength, mobility and independence. Plan and administer prescribed PT treatment in patients’ homes, assess and document physical function, develop and implement a rehabilitation program aligned with physician plans, supervise PTAs and Home Health Aides with clear care instructions, and communicate patient progress to the physician, Clinical Manager, care team, patient, and family. Requires current and unrestricted Physical Therapy license and at least one year of experience; collaborate with a healthcare professional team and maintain thorough progress notes and discharge summaries.
Required Qualifications
- Current and unrestricted Physical Therapy license
- One year of experience as a Physical Therapist
- Experience collaborating with a team of healthcare professionals
- Full compliance with home health care standards and documentation requirements
Additional Requirements
- Valid state driver license and personal vehicle liability insurance as required by Humana driver safety program
