Job Summary
Provide in-home physical therapy to patients, planning and administering prescribed PT treatment in patients' homes to improve functional outcomes. Assess and document patients' physical strengths to guide treatment goals, develop and implement rehabilitation programs aligned with physician plans, supervise PTAs and Home Health Aides with clear written care instructions, document observations and progress, and report to physician and care team while coordinating evaluations and discharge summaries. Requires active Physical Therapy license and at least one year of PT experience.
Required Qualifications
- Current and unrestricted Physical Therapy license
- One year of experience as a Physical Therapist
