Job Summary
In-home Physical Therapist role focusing on improving patients’ strength, mobility, and independence. Plan and administer prescribed PT treatment in patients’ homes, assess physical capabilities, develop and implement rehab programs aligned with physician plans, supervise PTAs and Home Health Aides with clear care instructions, document observations and progress, and report to physicians and care teams. Requires an accredited PT degree, current license, at least one year of PT experience, and a valid driver’s license with vehicle insurance; environment emphasizes collaboration with a multi-disciplinary team and delivering comprehensive home-based care.
Required Qualifications
- Degree from an accredited Physical Therapy Program (approved by the APTA)
- Current and unrestricted Physical Therapy license
- One year of experience as a Physical Therapist
- Valid state driver\'s license and ability to maintain personal vehicle liability insurance (via Humana driver safety program)
