Job Summary
Deliver personalized home-based physical therapy to patients in their homes, plan and administer prescribed PT treatments, assess and document physical strengths to guide treatment goals, develop and implement a rehabilitation program aligned with physician plans, supervise PTAs and Home Health Aides with clear care instructions, and promptly document observations and progress. Communicate patient progress to physicians, Clinical Manager, care team, patient, and family; submit evaluations, plans, progress notes, and discharge summaries. Requires current unrestricted Physical Therapy license, at least 1 year of PT experience, ability to work in a home-health setting, and a valid state driver’s license with vehicle liability insurance. Required Qualifications
