Job Summary
Provide in-home physical therapy to patients, plan and administer prescribed PT treatment in patients' homes to achieve highest possible physical function; assess and document patients' physical strengths to guide treatment goals; develop and implement a rehabilitation program aligned with physician plans and patient goals; oversee and give written care instructions to PTAs and Home Health Aides; promptly document observations, interventions, and evaluations; report progress to physician, Clinical Manager, care team, patient, and family, and submit evaluations, plans, progress notes, and discharge summaries; requires current unrestricted PT license and at least one year of PT experience; role is within Humana's driver safety program and requires a valid state driver's license and personal vehicle liability insurance.
Required Qualifications
- Current and unrestricted Physical Therapy license
- One year of experience as a Physical Therapist
- Experience collaborating with a team of healthcare professionals
